We are assisting a client in their search for an experienced Pediatric (RN) Case Manager who is driven to provide the best care to patients and families during those vulnerable and emotional final days, and someone who will carry that supportive care to families and loved ones left behind. The RN Pediatric Case Manager will provide skilled palliative and end of life care in the community setting. The RN Pediatric Case Manager is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.

QUALIFICATIONS:
- Completion of a professional nursing program accredited by the Board of Registered Nursing and a valid and current license to practice in California
- Minimum two (2) years of pediatric nursing experience in acute care facility, pediatric/neonatal intensive care, pediatric oncology, and/or hospice required
- Knowledge of hospice/concurrent care programs is preferred, but not required
- Ability to learn and assimilate our electronic medical record program
- Pediatric Hospice and Palliative Care Certification is desirable
- Demonstrated ability to function independently, with minimal supervisor, as well as in an interdisciplinary team setting
- Possess the ability to establish and maintain positive working relationships with both administrative and clinical staff.
- Must possess the ability to deal with difficult and complex situations
- Possess excellent time management, communication and organizational skills
- Ability to maintain strict confidentiality of sensitive data and inspire others
- Possess reliable transportation, which may be used in the course of work, and have a valid California drivers license, acceptable DMV driving record, and proof of auto insurance and current auto registration
