We are assisting a client in their search for a Patient Care Manager/Asst. Director of Nursing with Hospice and/or Home Health industry experience. The client is seeking a dynamic RN with a hospice heart to join their wonderful hospice team. Enjoy great benefits, competitive salaries, outstanding leadership, and corporate support. A minimum of three (3) years of hospice experience is required, along with a proven ability to manage a fast-paced, independent work schedule.
The Hospice PCM / ADON will be responsible for implementing company policy and mission by maintaining appropriate staffing levels, meeting set standards and goals to support their Hospice DCS/DON in the effective running of hospice operations and delivering outstanding patient care, while taking care of & supporting staff/care teams
Responsibilities:
- Supervision, orientation, mentoring, developing and on-going evaluation of all patient care staff who are engaged in the delivery of care
- Assures compliance with agency policies and procedures, state and federal laws, regulations and accreditation standards.
- Participation in the Manager on-call rotation
- Assist with talent acquisition to ensure adequate staffing is maintained
- Coordinate and facilitate IDT meetings
- Assist with quality assurance and performance improvement efforts
- Assist with patient case load, as needed
- Manage IDT process for multiple Physicians and Teams
QUALIFICATIONS:
- MUST have at least one (1) year of management experience
- MUST have at least two (2) years of Hospice Care experience
- Associate’s Degree in Nursing is required; Bachelor’s Degree preferred
- Valid RN Texas License
- CHPN preferred
- Valid Texas Driver’s License and Proof of Auto Insurance
