PT/Physical Therapist – Home Health

Seeking a Physical Therapist with experience in the Home Health industry – no exceptions!  Highly negotiable on compensation.

As Home Health Physical Therapist, this position provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.

What You Will Do in This Role:

  • You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
  • You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
  • You will provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
  • You will instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
  • You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

QUALIFICATIONS:

  • A current and unencumbered State Professional license as Physical Therapist
  • One (1) year experience as a home care professional physical therapist and competent in performing comprehensive assessment and OASIS-C preferred
  • The ability to make sound professional clinical judgment
  • The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
  • Proficiency in clinical skills
  • Excellent verbal and written communication skills and is able to read, write and comprehend English
  • An automobile to be used for work, current driver’s license, good driving record and proof of insurance
  • Proficiency in personal computer use, including email, clinical, word processing, and spreadsheet software
